REQUIREMENTS
PreK – 4 Year Old
Child must be four years old on or before Sept. 1, 2017 and one of the following:
- Unable to speak and comprehend the English language;
- Educationally disadvantaged (qualify for free or reduced lunch program);
- Homeless;
- Child of an active duty member of the armed forces of the United States;
- Child of a member of the armed forces who was injured or killed while on active duty; or
- Is or ever has been in the conservatorship of the Department of Family and Protective Services following an adversary hearing.
Parents Need to Bring the Following Information
- State Certified Birth Certificate
- Family Proof of Income
- Child’s Social Security Card
- Current Immunization Records
- Proof of Residence: Utility Bill
