This is the link to the UIL website for any questions you may have.
Kingsville ISD Athletic Physical/Emergency Paperwork is all online!! The ONLY thing to turn in the the actual physical to the HMK athletic office/training room.
Go to https://kingsvilleisd.rankonesport.com (DO NOT PUT A WWW.) Type it how it is written here. Click on the electronic forms tab. You will see the following forms:
___ UIL Forms: Acknowledgement of Rules, Concussion, Cardiac Awareness, Steroids
___ KISD Athletic Participation Insurance Information Form
___ Athletic Emergency Card
___ Medical History
Once you have completed each of the following forms, print off the Physical Form and take ONLY that form to the doctor to be completed. Once it is signed by the doctor you will bring the physical form to the Athletic Trainers office, or the main Athletic office. Do not take the doctors physicals to Memorial or the High School main offices, or the coaches please.
